Lorry, bus, coach and taxi drivers
Start with your driving licence: a lorry or bus licence goes to DVLA on form D2, and it is free. Your Driver CPC card, the DQC, uses the photo and signature from that licence, and GOV.UK publishes no separate form for a new name on it, so ask DVSA. A digital tachograph card changes by post on form D777B, which lists a change of name as free. A taxi or private hire licence belongs to the council that issued it, or to TfL in London.
Two come from DVLA, one from DVSA and one from your licensing authority, and only the first two have a published form for a new name.
You can keep driving while DVLA has it, according to GOV.UK.
The new card carries the photo and signature from your photocard licence.
It is built from your licence record and posted to the address on your licence.
In London, TfL takes a change of name by email.

The Driver CPC card, which GOV.UK also calls the driver qualification card or DQC, is made from your driving licence: it uses the photo and signature from your photocard licence, it is posted to the address on that licence, and it carries no address of its own. GOV.UK sets out no form or fee for a new name on it.
So change the licence first, then email DVSA Driver CPC enquiries at cpcre@dvsa.gov.uk with your driving licence number. Ask three things: how it issues a card in your new name, whether there is a fee, and what to carry in the meantime. You must carry the card whenever you drive a lorry, bus or coach professionally, and GOV.UK puts the fixed penalty for not having it at £50.
Once the new licence arrives, sign in to DVSA’s training record service with its licence number and check your hours are all there.
It belongs to the licensing authority that issued it, not to DVLA, so the rules are local. GOV.UK names the authority: your council outside London, and Transport for London in it.

Contact the licensing team that issued your badge. Ask what proof it takes, whether a certified copy will do, and whether a new licence or badge in your new name follows, at what fee.
GOV.UK’s find your council service gives the right one.
GOV.UK points to different processes for taxi and private hire licences in both. Ask the authority that licensed you in the same way.
A Northern Ireland licence, CPC card and tachograph card are the DVA’s, not DVLA’s.
For the part 1 theory test and the part 2 case studies test, GOV.UK says to bring proof of your change of name, such as your Deed Poll, along with the licence you take to every test.
Without that proof, cancel the test and book it again in your new name, using your new licence number. Turn up without the right documents and the test is cancelled and the fee lost.
From GOV.UK’s lorry and bus driver guidance, checked October 2026.
Change your driving licence first: form D2 to DVLA for a lorry or bus licence, which is free. The Driver CPC card, also called the DQC, uses your licence photo and signature and is posted to the address on your licence, and GOV.UK publishes no separate form for a new name. Then email DVSA Driver CPC enquiries at cpcre@dvsa.gov.uk and ask how it issues a card in your new name. What to ask DVSA.
No. DVLA's form D777B(DL) lists a change of name as free, sent to DVLA, Swansea, SA99 1ST. It is a postal application only, and the form asks you to put your driving licence application in the same envelope.
No. The form tells you not to return your card with a change of name application. Your new card comes with a confirmation of receipt letter, which you fill in and send back to DVLA.
For the part 1 theory test or the part 2 case studies test, take proof of your change of name, such as your Deed Poll, with your licence. Without it, GOV.UK says to cancel and book again in your new name with your new licence number, because a test taken without the right documents is cancelled and the fee is lost.
The authority that issued it: your local council outside London, or Transport for London in London. TfL takes a change of name by email at drivers@tph.tfl.gov.uk. A council sets its own process and any fee, so ask its licensing team what proof it wants. Taxi and private hire licences.
