If a name, the date or a signature can no longer be read, it is time for one. HM Passport Office’s caseworker guidance has its staff ask for a new deed when one is not signed in both names, witnessed and dated, and damage that hides any of those raises the same question. Lamination is a problem of its own: GOV.UK tells drivers not to send DVLA laminated certificates, and HM Passport Office turns laminated evidence down unless it is a change of name deed sent with a passport application. A replacement is £18.50 and records the same change and its original date.
